Topical finasteride, oral finasteride — and what the serum tells us

Finasteride works. Oral finasteride, taken daily, reliably lowers scalp and serum dihydrotestosterone and slows or reverses androgenetic hair loss in a large majority of men. The problem has never been whether it works; it is whether men will take it. A minority experience — or fear — sexual and mood-related side effects attributed to systemic DHT suppression, and that fear is the dominant reason prescriptions are abandoned.

The pharmacological question

This sets up a precise question. Finasteride’s therapeutic target is the scalp follicle, but its side-effect profile derives from systemic exposure. Could the drug be delivered topically — reaching the follicle in effective concentration — while keeping the amount entering the bloodstream, and therefore serum DHT, far lower? If so, you would decouple efficacy from the exposure patients fear.

What the review found

A systematic review synthesising the topical-versus-oral trials reported the encouraging answer: topical scalp finasteride produced comparable improvements in hair count and comparable local reductions in scalp DHT, while serum DHT suppression was markedly smaller than with oral dosing. The local pharmacology was preserved and the systemic footprint shrank. Trials varied in vehicle, concentration and duration, and longer studies are still needed — but the direction of the signal is consistent.

Why we take it seriously

We treat this as a design brief, not just a data point. If the biggest barrier to a proven therapy is a fear rooted in systemic exposure, the most valuable thing a formulator can do is deliver the same molecule to the same target with a fraction of that exposure. A precise, non-greasy scalp spray that puts finasteride where hair is falling — and largely nowhere else — is the practical expression of that idea.
